Simply copy and paste either of the codes and fill it out. It's pretty self explanatory from there. Haha. Also- I'll get to work on your signature tomorrow Hippo. c:

I'm not sure how to fill it out; what's with all the 's and 's? Is there a trick to this, or do you just want a list in that order? Do I just write in where it says [write here]?
