One of my favorites is the Kaiten. This was a human-steered torpedo. This was indeed used, arguably to little avail. Here's some history from a few sources.

The French navy built a 3,200-ton submarine with two 8-inch guns in a twin turret. Less a wonder weapon than a white elephant, the Surcouf mysteriously disappeared somewhere between Bermuda and the Panama Canal in 1942. Rumors abounded about the loyalties of her crew, allegedly split between Free French and Vichy, and the wreck and her crew have never been found.
