Once you conquer a country, do you have to worry about an uprising in those conquered provinces?

Yes, uprisings usually take place when the province morale is 25% or lower, and when you capture a province its morale starts at 25%. That's why you need to occupy the provincial center with a unit or units whose combined strength points exceed 7.5, until at least the first daily update following its capture. If you click on the "i" button next to provincial morale in the province's pop-up information box, it will show all of the factors that are affecting the province's morale, as well as the combined strength points of any occupying units. It also shows the percentage likelihood of a rebellion.

I get that is what normally happens....but if the country is defeated and no longer on the diplomacy page. Its provinces can revolt and put it back in the game?

Conceivably, yes. However, it's more likely the rebellious provinces will flip to another country still on the board, usually one that borders on the revolting province. That said, all of us who have played the game long enough have a weird anecdote or two in which a revolting province in India joins California, or a province in Spain joins Sweden. Weird, yes, but it does happen.
